Rilonacept (Arcalyst) is a subcutaneous IL-1 inhibitor used to treat Cryopyrin-Associated Periodic Syndromes (CAPS), including Familial Cold Autoinflammatory Syndrome (FCAS) and Muckle-Wells Syndrome (MWS). It works by blocking interleukin-1, a protein that drives inflammation in these rare auto-inflammatory disorders. Rilonacept is available as an 80 mg/mL solution in pre-filled syringes.

Product Description:

Rilonacept is an interleukin-1 trap: a fusion protein combining the extracellular domains of the IL-1 receptor and its accessory protein with the Fc portion of human IgG1. It acts as a decoy receptor, binding IL-1 beta — and also IL-1 alpha and the IL-1 receptor antagonist — with high affinity before they can reach cell-surface receptors. Its broader binding distinguishes it from antibodies specific to IL-1 beta alone.

It is used for cryopyrin-associated periodic syndromes, for deficiency of the IL-1 receptor antagonist, and for recurrent pericarditis, where it both treats the acute episode and reduces recurrence — an indication that expanded its market well beyond the rare autoinflammatory syndromes it was developed for. It is given by weekly subcutaneous injection after a loading dose. Because it blocks a central innate immune signal, serious infection risk rises, tuberculosis must be excluded before starting, and live vaccines are avoided. Injection site reactions are very common and lipid changes are reported.
