Cevimeline is a muscarinic receptor agonist that increases saliva and tear production. It is primarily prescribed to treat dry mouth (xerostomia) in patients with Sjögren’s syndrome, an autoimmune disease affecting moisture-secreting glands. It helps relieve symptoms of oral dryness and discomfort.

Cevimeline is a muscarinic receptor agonist with relative selectivity for the M3 subtype found on salivary and lacrimal gland cells. Stimulating those receptors makes the remaining functional glandular tissue secrete, increasing saliva and tear production. Its selectivity for M3 over M2 is intended to limit the cardiac and pulmonary effects that come with non-selective agonists such as pilocarpine.

It is used for dry mouth in Sjögren's syndrome, an autoimmune disease in which lymphocytes infiltrate and destroy the exocrine glands. Dry mouth there is not a minor complaint: it causes rampant dental caries, difficulty eating and speaking, and oral candidiasis. Cevimeline works only where some functional gland tissue survives, so it is less useful in advanced disease. Sweating is the commonest adverse effect, along with nausea and rhinitis. It is contraindicated in uncontrolled asthma, narrow-angle glaucoma and acute iritis, and used with caution in cardiac disease.
