Caplacizumab is a nanobody-based medication that targets von Willebrand factor (vWF), preventing platelet aggregation and microthrombi formation. It’s used alongside plasma exchange and immunosuppression to treat acquired thrombotic thrombocytopenic purpura (aTTP) in adults, helping reduce clot formation and disease recurrence.

Caplacizumab is a nanobody — a single-domain antibody fragment derived from camelid immunoglobulin — that binds the A1 domain of von Willebrand factor and blocks its interaction with platelet glycoprotein Ib. In acquired thrombotic thrombocytopenic purpura, autoantibodies destroy the enzyme ADAMTS13, so ultra-large von Willebrand factor multimers persist and spontaneously bind platelets, forming microthrombi that consume platelets and damage brain, heart and kidneys.

It is used with plasma exchange and immunosuppression in acquired TTP, where it acts immediately to stop new microthrombi forming while the immunosuppression works on the underlying antibody. In trials it shortened the time to platelet count recovery and reduced TTP-related death, recurrence and refractory disease. Because it blocks platelet adhesion, bleeding is the expected risk, and it is held before invasive procedures. Treatment continues for a period after plasma exchange ends, guided by ADAMTS13 activity, since stopping too early allows relapse.
