Pioglitazone is a thiazolidinedione and a PPAR-gamma agonist. It acts in adipose tissue, muscle and liver to increase insulin sensitivity by altering transcription of genes governing glucose and lipid metabolism — which is why the effect builds over weeks rather than days, unlike a secretagogue. It does not stimulate insulin secretion and so carries no intrinsic hypoglycaemia risk when used alone.

The safety profile is what has shaped its use. Fluid retention can precipitate or worsen heart failure and carries a boxed warning; it is contraindicated in NYHA class III and IV. Weight gain is expected. Distal fracture risk is raised, particularly in women.
