Product Description:

Glyburide, known as glibenclamide outside the US, is a second-generation sulfonylurea. It binds the SUR1 subunit of the ATP-sensitive potassium channel on pancreatic beta cells, closing the channel, depolarising the cell and triggering insulin release. Crucially, it does this regardless of the blood glucose level, which is the source of both its efficacy and its main danger.

It is used for type 2 diabetes, usually after metformin. It lowers HbA1c effectively and costs very little, which is why it remains widely used in low and middle income countries and appears on essential medicines lists. Its drawbacks are well documented: it causes more hypoglycaemia than other sulfonylureas, particularly in older patients and in renal impairment, because its active metabolites are renally cleared and accumulate. Hypoglycaemia from it can be prolonged and requires admission. It causes weight gain, and guidelines in wealthier markets now favour agents that do not cause hypoglycaemia.
