Diazoxide is used to treat hypoglycemia caused by excessive insulin production and is also used in emergency management of severe hypertension. It works by opening potassium channels, causing relaxation of smooth muscles in blood vessels, and by inhibiting insulin release from the pancreas, leading to an increase in blood glucose levels.

Diazoxide opens ATP-sensitive potassium channels, and the consequence depends entirely on which tissue the channel sits in. In the pancreatic beta cell, opening the channel holds the membrane polarised so calcium never enters and insulin is not released. In vascular smooth muscle the same action relaxes the vessel and lowers blood pressure. One molecule, two quite separate clinical uses, and two quite separate formulations.

The oral forms — tablets, capsules and suspension — are used for hyperinsulinaemic hypoglycaemia: congenital hyperinsulinism in infants, insulinoma, and other states of inappropriate insulin secretion. Treatment is often long term and started in hospital. Fluid retention is prominent and usually requires a thiazide diuretic alongside, which conveniently also raises blood glucose. Hypertrichosis — excess hair growth — occurs in most children on prolonged treatment and distresses families. Pulmonary hypertension has been reported in neonates.

Sourcing notes. A rare-disease product with small, steady, highly specialised demand concentrated in paediatric endocrinology units. The oral suspension matters disproportionately, since the patients are frequently infants. Note that the intravenous product for hypertensive emergency is a different formulation and has been withdrawn in most markets. Confirm which presentation is wanted.
