Product Description:

Ertugliflozin is an SGLT2 inhibitor. It blocks glucose reabsorption in the proximal renal tubule, so glucose is excreted in urine rather than returned to the blood. The mechanism is insulin-independent, and it brings the other effects of the class with it: modest weight loss from the calories lost in urine, and a fall in blood pressure from mild osmotic diuresis.

It is used for type 2 diabetes, alone or combined with metformin or sitagliptin in fixed-dose products. Its cardiovascular outcome trial demonstrated non-inferiority for safety but did not show the reduction in cardiovascular events that empagliflozin and canagliflozin achieved, which is why guidelines that specify an SGLT2 inhibitor for cardiovascular or kidney protection tend to name those rather than this one. Class adverse effects apply: genital mycotic infection, urinary tract infection, volume depletion and euglycaemic ketoacidosis, which must be considered in any unwell patient on the drug even with normal glucose.
