It works by replacing or supplementing endogenous insulin, facilitating glucose uptake into cells, and reducing blood sugar levels after meals. It is administered via subcutaneous injection and may also be given intravenously in hospital settings for rapid glycemic control.

Insulin glulisine is a rapid-acting insulin analogue. Two amino acid substitutions in the B chain stop the molecule forming the stable hexamers that slow absorption of human insulin from subcutaneous tissue, so it is absorbed as monomers within minutes. Onset is about ten to twenty minutes and the effect lasts three to five hours, roughly matching the glucose rise from a meal.

It is used at mealtimes in type 1 and type 2 diabetes, usually with a long-acting basal insulin, and in insulin pumps. Its practical advantage over human regular insulin is timing: it is injected immediately before eating, or even just after, which suits people with variable appetite or unpredictable meals, including children. Hypoglycaemia is the main risk, and it comes on faster than with regular insulin. Dose must be matched to carbohydrate intake and activity, and injection sites rotated to avoid lipohypertrophy, which makes absorption erratic.
