Product Description:

Epinephrine, called adrenaline in British practice, is the body's own fight-or-flight hormone given as a drug. It stimulates alpha and beta adrenergic receptors. It tightens blood vessels, strengthens the heartbeat and opens the airways. This page covers injection at two strengths. The 1 mg/mL solution, once written 1:1,000, is for intramuscular or subcutaneous use. The 0.1 mg/mL solution, once written 1:10,000, is for intravenous use in resuscitation.

In anaphylaxis the 1 mg/mL solution is given into the outer thigh muscle, with the dose repeated after five minutes if needed. In cardiac arrest the 0.1 mg/mL solution, often in a prefilled syringe, is given intravenously or intraosseously following resuscitation guidelines. Mixing up the two strengths has caused serious harm, so many countries now label in mg/mL only and keep the strengths apart on shelves. Pharmacists will expect warnings on arrhythmias, high blood pressure and tissue injury after leakage. Epinephrine is on the WHO Model List of Essential Medicines.

Sourcing notes. Epinephrine injection is a prescription medicine for hospital, clinic and emergency use. Ask for a CTD or ACTD dossier, sterility and endotoxin results on each batch CoA, and the GMP certificate of the sterile site with a CoPP. The solution oxidises in light and air and is often protected with a sulfite antioxidant; it must stay in the carton, and pink or brown solution must be discarded. Store at room temperature. Specify strength, ampoule, vial or prefilled syringe, and mg/mL labelling.
