Product Description:

Disopyramide is a class Ia antiarrhythmic. It blocks sodium channels, slowing conduction and prolonging the refractory period, and it also prolongs repolarisation. Separately it has strong anticholinergic activity and it reduces the force of cardiac contraction — properties that are drawbacks in most settings but useful in one.

Its traditional use is in ventricular and supraventricular arrhythmias. Its more distinctive modern use is in obstructive hypertrophic cardiomyopathy, where its negative inotropic effect reduces the outflow tract obstruction that causes breathlessness, making it a long-standing option alongside beta-blockers before invasive treatment or newer myosin inhibitors. Its limitations are considerable: anticholinergic effects cause dry mouth, constipation, blurred vision and urinary retention, particularly in older men with prostatic enlargement; it can precipitate heart failure; and it prolongs the QT interval with a risk of torsades de pointes, so it is started with monitoring. Renal and hepatic impairment require dose reduction.
