Atenolol is a beta-blocker that affects the heart and circulation (blood flow through arteries and veins). Atenolol is used to treat angina (chest pain) and hypertension (high blood pressure). Atenolol is also used to lower the risk of death after a heart attack.

Atenolol is a cardioselective beta-1 blocker and one of the most widely prescribed beta-blockers worldwide. It slows heart rate, reduces the force of contraction and lowers renin release, which reduces cardiac work and oxygen demand. It is water-soluble, so it crosses into the brain less than propranolol or metoprolol, which means fewer central effects such as vivid dreams and fatigue.

It is used for hypertension, angina, after myocardial infarction and for rate control in some arrhythmias. Its position in hypertension has weakened: trials found it less effective than other antihypertensives at preventing stroke, and guidelines no longer place beta-blockers first line for uncomplicated hypertension. It remains valuable where there is a specific indication, such as angina or post-infarction care. It is cleared by the kidneys, so the dose is reduced in renal impairment, and it is avoided in severe asthma, marked bradycardia and high-degree heart block. Abrupt withdrawal can provoke rebound angina.
