Product Description:

Colistimethate sodium is an inactive prodrug of colistin, a polymyxin antibiotic from the 1950s. After injection it hydrolyses to colistin, which binds lipopolysaccharide in the outer membrane of Gram-negative bacteria and disrupts it. Largely abandoned for decades because of kidney toxicity, it returned to use as a last-line option against multidrug-resistant Pseudomonas aeruginosa, Acinetobacter baumannii and carbapenem-resistant Enterobacterales.

It is given intravenously in severe infection and by nebuliser in chronic Pseudomonas infection in cystic fibrosis and bronchiectasis. Nephrotoxicity is dose-related and common, and neurotoxicity, with paraesthesia and confusion, occurs at higher exposures. Dosing must account for the lag while the prodrug converts, which is why a loading dose is used in critical illness.

Products are labelled either in international units of colistimethate or in milligrams of colistin base activity, and the two differ roughly threefold; confusion between them has caused serious overdoses.
