Product Description:

Cilnidipine is a dihydropyridine calcium channel blocker that blocks both L-type and N-type calcium channels. N-type channels sit on sympathetic nerve endings, and blocking them reduces noradrenaline release. The practical consequence is that cilnidipine lowers blood pressure without the reflex tachycardia typical of the class, and it may reduce the sympathetic overactivity that contributes to cardiovascular risk. N-type blockade also dilates the efferent arteriole in the kidney, lowering glomerular pressure, which underlies its reputation for reducing proteinuria.

It is used once daily for hypertension, and it is often chosen for patients with chronic kidney disease and proteinuria or with palpitations on amlodipine. Comparative trials against amlodipine have shown less ankle oedema and less protein in urine, although hard outcome data are limited. Adverse effects are those of the class, generally mild. It is metabolised by CYP3A4.
