Candesartan is a medicine used to treat high blood pressure and heart disease. Lowering blood pressure helps to prevent future heart attack and stroke. This medicine is also effective in preserving kidney function in patients with diabetes.

Product Description:

Candesartan cilexetil is a prodrug rapidly converted during absorption to candesartan, an angiotensin II receptor blocker. It binds the AT1 receptor tightly and comes off slowly, which gives a long duration of action and consistent 24-hour blood pressure control from once-daily dosing. Because it blocks the receptor rather than the converting enzyme, it does not raise bradykinin, so it rarely causes the dry cough associated with ACE inhibitors.

It is used for hypertension and for heart failure with reduced ejection fraction, where the CHARM programme showed it reduced cardiovascular death and heart failure hospitalisation, including in patients intolerant of ACE inhibitors. It is also used to prevent migraine in some markets, and after stroke in others. Potassium and renal function are monitored, particularly in older patients and those on diuretics. It is contraindicated in pregnancy, and it should not be combined with an ACE inhibitor or aliskiren because dual blockade increases harm without benefit.

Sourcing notes. Off patent, with generics widely registered. The cilexetil ester is moisture-sensitive, so packaging and stability data matter. Fixed-dose combinations with hydrochlorothiazide and with amlodipine are separate registrations.
