Betaxolol is a selective beta-1 adrenergic receptor blocker (beta-blocker) used to treat high blood pressure (hypertension) and angina (chest pain). It works by reducing heart rate and cardiac output, helping the heart to pump more efficiently and lowering blood pressure.

Betaxolol is a cardioselective beta-1 blocker used in two quite different ways. As an oral tablet it lowers blood pressure by reducing heart rate, cardiac output and renin release, with a long half-life that supports once-daily dosing. As an ophthalmic solution it lowers intraocular pressure by reducing the production of aqueous humour in the ciliary body, and is used for open-angle glaucoma and ocular hypertension.

In glaucoma its cardioselectivity is the point: timolol, the older beta-blocker eye drop, is non-selective and can cause bronchospasm when absorbed systemically, so betaxolol is preferred for patients with airway disease, although it lowers pressure slightly less. Even eye drops are absorbed enough to cause systemic effects, so pressing on the tear duct after instilling reduces absorption. Oral use carries the usual beta-blocker cautions: bradycardia, heart block, decompensated heart failure and caution in asthma. Abrupt withdrawal of the tablet can provoke rebound effects.

Sourcing notes. The tablet and the eye drop are separate products with separate registrations, manufacturing requirements and buyers; the eye drop is a sterile ophthalmic preparation. Buyers should state which is required. Both are off patent with generics available.
