Anastrozole is a medicine used to treat breast cancer in women who have gone through menopause. It adjusts the level of sex hormone (estrogen) to treat early cases of breast cancer and helps in slowing down the growth of cancer cells, that need estrogen to grow.

Anastrozole is a non-steroidal aromatase inhibitor. In postmenopausal women the ovaries no longer make oestrogen, but the aromatase enzyme in fat, muscle and breast tissue converts adrenal androgens into it, and that residual oestrogen drives hormone receptor-positive breast cancer. Anastrozole blocks aromatase reversibly and suppresses circulating oestrogen by more than ninety percent.

It is used as adjuvant treatment for early hormone receptor-positive breast cancer in postmenopausal women, and for advanced disease. It has largely replaced tamoxifen in this population, having shown better disease-free survival and fewer endometrial cancers and thromboembolic events — though with a different toxicity profile. The trade-off is skeletal and musculoskeletal: oestrogen deprivation accelerates bone loss, so bone density is monitored and bisphosphonates are often added, and arthralgia affects a large minority and is the commonest reason for stopping. It is ineffective in premenopausal women, whose ovaries simply increase output in response.
