Product Description:

Aprepitant blocks the neurokinin-1 receptor, where substance P acts. Chemotherapy-induced vomiting has two phases: an acute phase in the first day, driven largely by serotonin, and a delayed phase over the following days, driven by substance P. 5-HT3 antagonists control the first and do little for the second, which is why the addition of an NK1 antagonist changed the management of highly emetogenic chemotherapy.

It is used to prevent nausea and vomiting from cisplatin-based and other highly emetogenic regimens, and from moderately emetogenic chemotherapy, given with a 5-HT3 antagonist and dexamethasone as a three-drug regimen over three days. Preventing vomiting matters clinically as well as humanely: uncontrolled emesis causes dehydration, treatment delay, and anticipatory nausea that is very difficult to treat once established. It is a moderate CYP3A4 inhibitor, so the dexamethasone dose is reduced when given with it, and it interacts with several chemotherapy agents.
