If you work in pharma trade, you hear the same names again and again. Sun, Cipla, Takeda, Roche, Pfizer. But who are they really, where are they based, and can a buyer or distributor actually do business with them? Here are 35 of the big ones, in plain words, and what they mean for you.

First, how we picked them
This isn't a "biggest by sales" ranking. We picked companies that buyers, distributors and licensing partners actually run into, whether because their generics are everywhere, because of what's in their pipelines, or because they make medicines for other companies.
One thing to keep in mind: we list each company under the country where its head office is. Most of them have factories in several countries, so "based in Switzerland" doesn't mean "made in Switzerland".
Asia
1. Sun Pharmaceutical Industries: India
Known for: generics, specialty medicines, skin care
If you've bought generics from India, you've probably come across Sun. It's India's biggest drug company by revenue and sells in the US, India and many emerging markets. Lately it has been putting more into specialty products, mainly skin and eye care.
2. Cipla: India
Known for: inhalers, HIV medicines, generics
Cipla became famous for bringing cheap HIV medicines to Africa. Today it's one of the biggest makers of generic inhalers, and it's strong in India, South Africa and the US.
3. Dr. Reddy's Laboratories: India
Known for: generics, APIs, biosimilars
Based in Hyderabad. Dr. Reddy's sells finished medicines worldwide and also sells APIs to other manufacturers, so it's worth knowing whichever of the two you're buying.
4. Zydus Lifesciences: India
Known for: generics, biosimilars, vaccines
You may still know it as Zydus Cadila, the name it used until 2022. It's based in Ahmedabad and makes a wide range of generics, as well as biosimilars and its own vaccines.
5. Aurobindo Pharma: India
Known for: generics, APIs, injectables
By volume, one of the biggest suppliers of generics to the US. Aurobindo makes most of its own APIs, which helps it keep costs down, and it has a large injectables business.
6. Lupin: India
Known for: generics, inhalers, TB medicines
A Mumbai company that's strong in US generics, respiratory products and tuberculosis treatment. At home in India it also has a big branded business.
7. WuXi AppTec: China
Known for: making and developing medicines for other companies
WuXi doesn't sell its own brands. Drug companies hire it to do research, development and manufacturing for them. If you're in the US, check where the latest US biotech-security rules stand before you sign a long-term deal with any Chinese contract manufacturer.
8. Jiangsu Hengrui Pharmaceuticals: China
Known for: cancer medicines, new drugs, generics
One of China's biggest drug makers, and one of the biggest spenders on research there. Western companies are now licensing Hengrui's new molecules, which says a lot about how far Chinese pharma has come.
9. Takeda: Japan
Known for: gut medicines, rare diseases, plasma products
Japan's biggest drug company. When it bought Shire in 2019, it picked up a large rare-disease and plasma business to go with its gut and cancer medicines.
10. Astellas Pharma: Japan
Known for: cancer, urology
Tokyo-based. Most people know Astellas for its prostate cancer and overactive bladder medicines. It's now pushing into cell and gene therapy too.
11. Eisai: Japan
Known for: brain disorders, cancer
Eisai developed the Alzheimer's medicine lecanemab together with Biogen. It also makes the cancer medicine lenvatinib.
12. Shionogi: Japan
Known for: infections
An Osaka company that focuses mainly on infections, including new antibiotics for bugs that resist older ones, and antivirals.
13. Samsung Biologics: South Korea
Known for: making biologics for other companies
Yes, that Samsung. Its biologics arm has some of the largest capacity in the world for making antibodies and other biologics for drug companies, in huge plants at Incheon.
14. Celltrion: South Korea
Known for: biosimilars
One of the first companies to get a biosimilar antibody approved in Europe, and still one of the big names in biosimilars.
Europe
15. Roche: Switzerland
Known for: cancer, immunology, diagnostics
One of the world's biggest biotech companies (Genentech in the US is part of Roche). It's also a leader in lab tests and diagnostics.
16. Novartis: Switzerland
Known for: heart, cancer, immunology, brain disorders
Novartis used to own Sandoz. Since spinning it off in 2023, Novartis has worked only on new medicines, not generics.
17. Sandoz: Switzerland
Known for: generics, biosimilars
The old version of this article listed Sandoz under India. It's actually Swiss, headquartered in Basel, and has been independent since October 2023. It's one of the world's biggest generics and biosimilars companies and a major antibiotics maker in Europe.
18. Sanofi: France
Known for: immunology, vaccines, rare diseases
Paris-based. Sanofi is one of the world's biggest vaccine makers and sells the eczema and asthma medicine Dupixent together with Regeneron.
19. Bayer: Germany
Known for: heart, cancer, women's health
Bayer also works in crop science and consumer health. In medicines it's best known for blood thinners, women's health and radiology.
20. Merck KGaA: Germany
Known for: cancer, fertility, lab and production supplies
This is the German Merck, from Darmstadt, not the American one (that's no. 28). In the US and Canada it goes by EMD. Many drug makers also buy their materials and equipment from it.
21. Boehringer Ingelheim: Germany
Known for: lungs, heart and diabetes, animal health
Still family-owned, which is unusual for a company this big. It also makes biologics for other companies.
22. AstraZeneca: United Kingdom
Known for: cancer, heart and diabetes, lungs, rare diseases
A British-Swedish company headquartered in Cambridge. When it bought Alexion, it gained a big rare-disease business.
23. GSK: United Kingdom
Known for: vaccines, HIV (through ViiV Healthcare), lungs
Once called GlaxoSmithKline. It split off its consumer brands as Haleon in 2022 and now sticks to medicines and vaccines.
24. Hikma Pharmaceuticals: United Kingdom and Jordan
Known for: generic injectables, branded generics in the Middle East and North Africa
Hikma started in Amman and is now headquartered and listed in London. It's a major supplier of generic injectables to US hospitals and one of the biggest drug companies in the Middle East and North Africa.
25. Novo Nordisk: Denmark
Known for: diabetes, weight loss
The insulin company behind semaglutide. That patent is now running out in more and more countries, and generics are following. Here's where generic semaglutide is legal, country by country.
United States
26. Pfizer
Known for: vaccines, cancer, everyday medicines
Everyone knows Pfizer. What fewer people know is that it also makes medicines for other companies, through its contract manufacturing arm, Pfizer CentreOne.
27. Johnson & Johnson
Known for: immunology, cancer, brain disorders, medical devices
The shampoo and baby-powder side is gone: it became Kenvue in 2023. Today J&J is just two businesses, medicines and medical devices.
28. Merck & Co. (MSD)
Known for: cancer, vaccines
Outside the US and Canada you'll see it as MSD. Its cancer medicine Keytruda (pembrolizumab) is one of the best-selling medicines in the world.
29. Eli Lilly
Known for: diabetes, weight loss, brain disorders
Lilly has made insulin for about a century. Its tirzepatide medicines for diabetes and weight loss have turned it into one of the most valuable drug companies anywhere.
30. AbbVie
Known for: immunology, brain disorders, aesthetics
AbbVie grew on Humira and now sells its successors, Skyrizi and Rinvoq. Buying Allergan brought in Botox.
31. Bristol Myers Squibb
Known for: cancer, blood disorders, heart
Best known for the cancer immunotherapy Opdivo (nivolumab). It sells the blood thinner Eliquis (apixaban) together with Pfizer.
32. Amgen
Known for: biologics, bones, heart, biosimilars
One of the first biotech companies. Unusually for a company that invents new medicines, it sells biosimilars as well.
33. Gilead Sciences
Known for: HIV, liver disease, cancer
If it's about HIV, Gilead is usually in the picture, for both treatment and prevention. Its Kite unit makes CAR-T cell therapies.
34. Regeneron
Known for: eyes, immunology, cancer
The company behind the eye injection Eylea (aflibercept). It also developed Dupixent together with Sanofi.
35. Viatris
Known for: generics, older brand-name medicines
The old version of this article listed it as Mylan, in India. Viatris was formed in 2020 when Mylan merged with Pfizer's Upjohn business. It's a US company, although a lot of its manufacturing is in India.
So, can you actually buy from them?
Sometimes, but usually not the way people hope. A company this size won't often take a small order from a distributor it doesn't know. Here's what tends to work instead:
- Go through their local office. For brand-name medicines, you normally buy through the company's own office or its appointed distributor in your country. If there isn't one, ask whether you could be it.
- Look at their generics and tenders. The Indian companies, and Hikma, Sandoz and Viatris, sell generics widely, often through tenders and distributors.
- Ask about licensing. Plenty of these companies will license a dossier or a brand for a country they don't plan to enter themselves. Here's how those deals are usually priced.
- Need something made? Go to a contract manufacturer. A big innovator is usually the wrong door for that. Start with CDMO companies in India or how to choose a CDMO.
Whoever you buy from, check the paperwork before you pay: the GMP certificate, the CoPP (certificate of pharmaceutical product) and a CoA for each batch. Here's how to check a supplier. And if someone says they represent one of these big names but can't show you a signed authorisation letter, walk away.
