Voclosporin is an oral calcineurin‑inhibitor immunosuppressant used to treat active Lupus Nephritis (LN) in adults, typically in combination with a background immunosuppressive regimen. It works by inhibiting the enzyme calcineurin — thereby suppressing T‑cell activation, reducing inflammatory cytokine production, and helping stabilize kidney podocytes to reduce proteinuria and preserve kidney function.

Product Description:

Voclosporin is a calcineurin inhibitor, a modified form of ciclosporin. A small change to the molecule makes it more potent, so less drug is needed, and alters its metabolism so that blood levels are more predictable — which is why it is dosed by weight without the routine blood level monitoring ciclosporin requires. Inhibiting calcineurin blocks T-cell activation, and it also stabilises podocytes in the kidney directly, which may reduce protein loss independently of immune suppression.

It is used with mycophenolate and corticosteroids for active lupus nephritis in adults. In its pivotal trial the triple regimen roughly doubled the rate of complete renal response at one year compared with mycophenolate and steroids alone. The class risks apply: it constricts renal arterioles and can reduce kidney function, raises blood pressure, and increases infection risk, so blood pressure and creatinine are monitored and the dose reduced or stopped if function declines. It is a CYP3A4 substrate, so strong inhibitors are contraindicated.
