Product Description:

Methimazole, known as thiamazole in much of Europe, is a thionamide antithyroid drug. It inhibits thyroid peroxidase, the enzyme that adds iodine to tyrosine residues on thyroglobulin and couples them into thyroid hormone. Blocking that step stops new hormone synthesis, although stored hormone continues to be released, so the clinical effect takes several weeks.

It is first-line medical treatment for hyperthyroidism, particularly Graves' disease, and is used to restore normal thyroid function before surgery or radioiodine. It is more potent and longer-acting than propylthiouracil, allowing once-daily dosing, and causes less liver injury, which is why guidelines prefer it — with one exception: in the first trimester of pregnancy propylthiouracil is preferred, because methimazole is associated with a specific pattern of birth defects. The serious risk is agranulocytosis, rare but sudden, so patients are told to stop the drug and seek an urgent blood count if they develop fever or sore throat. Rash, joint pain and mild liver enzyme rises are more common.
