Product Description:

These tablets join two oral glucose-lowering drugs. Vildagliptin inhibits dipeptidyl peptidase-4 (DPP-4), the enzyme that breaks down the gut hormones GLP-1 and GIP, so after meals more insulin and less glucagon are released. Metformin, a biguanide, cuts liver glucose output and improves insulin sensitivity. Each film-coated tablet holds vildagliptin 50 mg with metformin 500, 850 or 1000 mg. The combination is approved in the EU and many other markets; vildagliptin is not approved in the US.

It is indicated for adults with type 2 diabetes not controlled on metformin alone, or already taking both drugs. The usual dose is one tablet twice daily with or after meals. Liver tests are advised before starting and at intervals, and it is not used in hepatic impairment. Metformin needs kidney checks and is paused around iodinated contrast and in acute illness because of lactic acidosis risk. Long-term metformin use can lower vitamin B12 levels.

A CTD dossier with bioequivalence against the reference product is standard for regulated markets, with ACTD in parts of Asia. Metformin raises a nitrosamine question, so ask for the manufacturer's nitrosamine risk assessment and test results.
