Succimer is meso-2,3-dimercaptosuccinic acid, an orally active heavy metal chelator. It is a water-soluble analogue of dimercaprol, and that single change is what makes it useful: chelation that previously meant painful intramuscular injection or intravenous infusion can be given by mouth, which matters when the patient is a child and the course runs 19 days.
Its principal use is lead poisoning in children at blood lead concentrations above the treatment threshold, where it forms water-soluble complexes excreted in urine. It also binds arsenic and mercury. Monitoring is for neutropenia and transient transaminase rise, and gastrointestinal upset and a sulphurous odour are common and expected rather than signs of a problem.
