Naloxone injection is the standard antidote for opioid overdose. Naloxone is a pure opioid antagonist: it pushes opioids such as morphine, heroin or fentanyl off their receptors and reverses breathing depression within minutes. It can be given into a vein for the fastest effect, or into a muscle or under the skin when a vein is hard to reach. Naloxone is on the WHO Model List of Essential Medicines.

Emergency teams, anaesthetists and ward staff use it for suspected opioid overdose and for opioid-induced breathing problems after surgery. Small doses are given and repeated every few minutes until breathing recovers. Its action is shorter than that of many opioids, so patients can slip back into overdose; they need observation and sometimes repeat doses or an infusion. In people who are opioid-dependent it can bring on sudden withdrawal. Buprenorphine needs higher or repeated doses to reverse.

Sourcing notes. Naloxone itself is not a controlled drug. It is a prescription medicine in most countries, though several now allow supply without prescription for overdose programmes. Ask for a CTD or ACTD dossier, sterility and bacterial endotoxin results on each batch CoA, the GMP certificate of the sterile site and a CoPP.
