Lomitapide is a microsomal triglyceride transfer protein (MTP) inhibitor used to lower LDL cholesterol in adults with homozygous familial hypercholesterolemia (HoFH). It decreases lipoprotein assembly in the liver, leading to reduced LDL, total cholesterol, and non-HDL cholesterol.

Product Description:

Lomitapide inhibits microsomal triglyceride transfer protein, the enzyme that loads fats onto apolipoprotein B inside liver and intestinal cells. Without that loading step, the liver cannot assemble and release VLDL, the precursor of LDL. Because it acts before the LDL receptor is involved, it lowers LDL cholesterol in homozygous familial hypercholesterolaemia, where patients have few or no working LDL receptors and statins and PCSK9 inhibitors work poorly or not at all.

It is used as an addition to other lipid-lowering treatment, including apheresis, in that rare condition, and it can lower LDL by around half. The trade-off is direct: blocking fat export from the liver causes fat to accumulate there, so hepatic steatosis and raised transaminases are expected, and progression to steatohepatitis or fibrosis is the long-term concern. Liver tests are monitored, alcohol is limited, and a low-fat diet with fat-soluble vitamin supplements is required to control gastrointestinal effects.
