Mexiletine is an oral class IB antiarrhythmic drug that helps stabilize abnormal heart rhythms by affecting sodium channels in cardiac muscle cells. It is most commonly used for managing ventricular arrhythmias, especially in patients with life‑threatening irregular heartbeats when other treatments are inadequate. Because it influences cardiac electrical activity, mexiletine is prescribed under close medical supervision. Common side effects may include nausea, dizziness, tremors, and ataxia.

Product Description:

Mexiletine is a class Ib antiarrhythmic, structurally related to lidocaine but orally active. It blocks sodium channels with fast binding and unbinding kinetics, so it acts preferentially on tissue that is depolarised or firing rapidly, shortening the action potential and suppressing ectopic activity while leaving normal conduction largely untouched.

It is used for symptomatic ventricular arrhythmias, and increasingly for two other purposes: non-dystrophic myotonias and myotonic dystrophy, where it reduces muscle stiffness and has trial support, and long QT syndrome type 3, where the underlying defect is a sodium channel that fails to close. It is also used off label in neuropathic pain. The class carries a serious caution: trials of similar drugs after myocardial infarction showed increased mortality, so use in structural heart disease is restricted to life-threatening arrhythmias. Nausea, tremor, dizziness and ataxia are common and dose-limiting; taking it with food helps.

Sourcing notes. Off patent but with a narrow supplier base, and it has been in shortage. In some markets it is available only through named-patient routes, particularly for myotonia. Capsules are the standard form, and demand comes from neuromuscular clinics as well as cardiology.
