Isradipine is a dihydropyridine calcium channel blocker. It binds L-type calcium channels in vascular smooth muscle, reducing calcium entry so the vessel relaxes and peripheral resistance falls. Within the dihydropyridine class it is relatively vascular-selective, with little direct effect on cardiac contractility or conduction — which distinguishes the class from verapamil and diltiazem and makes it safer to combine with beta blockade.

It is used for hypertension, alone or with other agents, and has been studied in Parkinson's disease on the hypothesis that reducing calcium load in dopaminergic neurons might be neuroprotective — a trial that did not show benefit but which keeps the molecule of research interest. The adverse effects are those of vasodilation: ankle oedema, flushing, headache, dizziness and palpitations. Oedema is dose-related and is not relieved by diuretics, since its cause is capillary pressure rather than fluid overload.
