Product Description:

Guanfacine is a selective alpha-2A adrenergic agonist. Stimulating these receptors in the brainstem reduces sympathetic outflow, which lowers blood pressure and heart rate — the original use. The second use came later and from a different receptor population: alpha-2A receptors in the prefrontal cortex, where they strengthen the network connections underlying working memory and impulse control. Enhancing that signalling improves attention without any stimulant effect.

The immediate-release tablet is used for hypertension, usually as add-on therapy. The extended-release tablet is used for attention deficit hyperactivity disorder in children and adolescents aged six to seventeen, alone or alongside a stimulant, and is particularly useful where tics, anxiety or sleep disturbance make stimulants difficult. Somnolence, sedation, fatigue, hypotension and bradycardia are the common effects; abrupt withdrawal after prolonged use can cause rebound hypertension, so the dose is tapered.

Sourcing notes. The two formulations are separate products with different dosing, different licensed indications and no milligram equivalence, and must never be substituted for one another.
