Product Description:

Fomepizole, or 4-methylpyrazole, competitively inhibits alcohol dehydrogenase, the liver enzyme that starts the breakdown of ethylene glycol and methanol. Their metabolites, glycolic and oxalic acid and formic acid, cause the acidosis, kidney failure and blindness of poisoning; blocking the enzyme lets the parent alcohol clear unchanged. It is a sterile concentrate for dilution and intravenous infusion. It holds 1 g per mL, usually 1.5 g in a 1.5 mL vial, and is diluted before infusion. It solidifies below about 25 °C and is liquefied by warming in the hand.

It is a hospital antidote for suspected or confirmed ethylene glycol or methanol poisoning, and has largely replaced ethanol infusion where available because dosing is simpler and it does not intoxicate. A weight-based loading dose is followed by doses every 12 hours, each diluted in at least 100 mL of sodium chloride 0.9% or glucose 5% and infused over 30 minutes. It induces its own metabolism, so doses rise after 48 hours, and dialysis removes it, so dosing is more frequent then. Headache and nausea are common.

Sourcing notes. Fomepizole is a prescription-only antidote held by hospitals and poison centres for rare, urgent cases, so long remaining shelf life matters more than volume. Ask for the CTD or ACTD dossier, a CoA per batch with sterility and endotoxin results, the GMP certificate and a CoPP. Vials are kept at room temperature in the carton, and staff in cool stores should know a solid vial is not spoiled.
