Flutamide is a nonsteroidal anti-androgen used primarily in the treatment of prostate cancer. It works by blocking androgen receptors, thereby inhibiting the action of male hormones (androgens) that stimulate the growth of prostate cancer cells. Flutamide is often used in combination with luteinizing hormone-releasing hormone (LHRH) agonists or other hormone therapies to enhance efficacy in managing advanced or metastatic prostate cancer.

Product Description:

Flutamide is a non-steroidal antiandrogen that competes with testosterone and dihydrotestosterone for the androgen receptor without activating it. Its historical role was to cover the testosterone surge caused by starting an LHRH agonist, and to provide combined androgen blockade by neutralising the adrenal androgens that castration leaves untouched. It has no hormonal activity of its own, so it does not suppress libido as completely as surgical or medical castration.

It is used in advanced prostate cancer, generally alongside an LHRH agonist, dosed three times daily because of a short half-life. The overriding safety concern is hepatotoxicity: fulminant hepatic failure has occurred, sometimes fatally, usually within the first few months, and liver function must be measured before treatment, monthly for the first four months, and periodically thereafter. Gynaecomastia, diarrhoea and a characteristic amber discolouration of urine are common.

Sourcing notes. Off patent and inexpensive, but largely superseded in well-resourced markets by bicalutamide, which is once-daily and far safer for the liver — demand now concentrates in price-sensitive markets. The mandatory liver monitoring schedule is a real barrier where laboratory access is limited. Oral solid, room temperature.
