Ferric citrate is a phosphate binder that does two jobs at once. Taken with meals, the ferric iron binds dietary phosphate in the gut to form insoluble ferric phosphate, which is excreted, lowering serum phosphate in chronic kidney disease. At the same time a portion of the iron is absorbed, raising iron stores — unusual for a binder, and clinically useful because dialysis patients are commonly iron deficient.

It is used to control serum phosphate in dialysis patients and, in some markets, to treat iron deficiency anaemia in chronic kidney disease not on dialysis. In trials it raised ferritin and transferrin saturation and reduced the need for intravenous iron and erythropoiesis-stimulating agents, which is a meaningful cost offset. Discoloured, dark stools are expected and harmless. Diarrhoea, constipation and nausea are common. Iron overload is possible with prolonged use, so iron studies are monitored, and it should not be used in patients with iron overload disorders.

Sourcing notes. The strength is stated as both tablet weight and ferric iron content, and both belong on any quotation, since prescribing is by iron content. Tablets are large and the pill burden is high, which affects adherence. Registration outside the US and Japan is still limited.
