Dimethylpolysiloxane, also known as dimeticone and, in its activated form with silicon dioxide, as simeticone, is a silicone antifoaming agent. It is not absorbed. It works physically rather than pharmacologically: by lowering the surface tension of gas bubbles trapped in gut contents, it causes small bubbles to merge into larger ones that can be passed by belching or as flatus.

It is used for flatulence, abdominal distension and postprandial or postoperative discomfort from trapped gas, and it is widely given before endoscopy and abdominal ultrasound to disperse bubbles that obscure the view. Because it is not absorbed and has no systemic activity, it is very safe, with essentially no interactions — although it may reduce absorption of levothyroxine if taken at the same time. Evidence for benefit in functional bloating is modest.

Sourcing notes. Chewable tablets are standard, since the silicone must disperse to work, and the tablet must be chewed rather than swallowed whole. The pharmaceutical specification depends on viscosity grade and, for simeticone, silicon dioxide content, so the pharmacopoeial monograph and grade should be named in any quotation.
