Product Description:

Degarelix blocks the GnRH receptor in the pituitary directly, as an antagonist. That mechanism matters because the older LHRH agonists first stimulate the receptor before downregulating it, producing a testosterone surge in the opening fortnight that can worsen bone pain, urinary obstruction or spinal cord compression in men with heavy disease. Degarelix suppresses testosterone to castrate levels within three days with no flare and therefore no need for antiandrogen cover.

It is used for advanced hormone-dependent prostate cancer, given as a monthly subcutaneous depot into the abdominal wall. The first dose is a 240 mg loading dose administered as two 120 mg injections, followed by 80 mg monthly. Injection site reactions — pain, erythema and induration — are common with the loading dose and diminish thereafter. Hot flushes, weight gain, raised transaminases and the metabolic and bone consequences of androgen deprivation follow the class.
