Cinacalcet is used to treat increased amounts of a certain hormone (parathyroid) in people with long-term kidney disease who are on dialysis. It is also used to treat increased amounts of calcium in people with an overactive parathyroid gland or in people with cancer of the parathyroid gland.

Product Description:

Cinacalcet is a calcimimetic. It binds the calcium-sensing receptor on parathyroid cells and makes it more sensitive to circulating calcium, so the gland behaves as if calcium were higher than it is and reduces parathyroid hormone secretion. It is the only class that lowers PTH while lowering calcium as well, unlike vitamin D analogues, which raise both calcium and phosphate.

It is used for secondary hyperparathyroidism in patients on dialysis, and for hypercalcaemia in parathyroid carcinoma and in primary hyperparathyroidism where surgery is not possible. In dialysis patients it reliably lowers PTH, calcium and phosphate; a large outcome trial did not meet its primary endpoint for death or cardiovascular events, so it is used for biochemical control rather than a proven survival benefit. Hypocalcaemia is the main risk, with paraesthesia, cramps and QT prolongation, so calcium is monitored. Nausea and vomiting are common and dose-limiting.

Sourcing notes. Off patent, with generics in the US, Europe and India. Tablets must be taken whole with food. The intravenous calcimimetic used during dialysis sessions is a different molecule and a separate product. Demand comes from dialysis programmes, so tender volumes can be large and predictable.
