Product Description:

Romiplostim is a thrombopoietin receptor agonist. It is a peptibody, an antibody Fc fragment fused to receptor-binding peptides, made by recombinant DNA technology in E. coli. Activating the receptor on megakaryocytes raises platelet production. It comes as lyophilised powder in a single-use vial, reconstituted with sterile water for subcutaneous injection. The 250 microgram vial is listed here. Reference vials carry an overfill and give a 500 microgram per mL solution once reconstituted with sterile water.

It is used for immune thrombocytopenia (ITP) in adults and children when corticosteroids, immunoglobulins or splenectomy have not worked well enough, and in the US also after marrow-suppressing radiation exposure. It is injected weekly, dosed by body weight and adjusted by platelet count, using a syringe graduated to 0.01 mL. The aim is a safe count, not a normal one. Platelets can drop after stopping, so counts are watched for at least two weeks. Bone marrow reticulin may increase, and myelodysplastic thrombocytopenia is excluded.

Unopened vials are kept at 2-8 °C in the carton, away from light and never frozen, so a recorded cold chain is needed.
