Product Description:

NPH insulin is human insulin crystallised with protamine and zinc. The protamine slows dissolution from the injection site, converting a short-acting insulin into an intermediate-acting one: onset in one to two hours, a distinct peak at four to eight hours, and a duration of twelve to eighteen. It is produced by recombinant DNA technology and is structurally identical to human insulin.

It is used as basal insulin in type 1 and type 2 diabetes, usually once or twice daily, and it is the basal insulin most of the world can afford. Compared with long-acting analogues it has a pronounced peak, which causes more nocturnal hypoglycaemia, and more variability in absorption between days. It is a suspension, not a solution, so the vial or pen must be gently resuspended before every dose; failure to do so is a common and significant cause of erratic control.

Sourcing notes. The 40 U/mL and 100 U/mL concentrations both exist in different markets, and confusing them is a classic, sometimes fatal dispensing error — the concentration must be unmissable on carton, vial and any quotation, and syringes must match.
