Letrozole is a non-steroidal aromatase inhibitor. After menopause the ovaries stop producing oestrogen, but small amounts are still made by the aromatase enzyme converting androgens in fat, muscle and the tumour itself — enough to sustain a hormone-receptor-positive breast cancer. Letrozole blocks that enzyme reversibly and reduces circulating oestrogen by more than ninety-five per cent, which is why it outperforms tamoxifen in postmenopausal women.

It is used as adjuvant treatment for early hormone receptor-positive breast cancer, for advanced disease, and increasingly with CDK4/6 inhibitors. It has a second and quite different use: in premenopausal women it triggers ovulation by removing oestrogen's negative feedback on the pituitary, and in polycystic ovary syndrome it now outperforms clomiphene for live birth rates. Adverse effects reflect oestrogen deprivation — hot flushes, arthralgia severe enough to cause many women to stop, vaginal dryness, and progressive bone loss requiring densitometry.

Sourcing notes. Long off patent and widely multi-source, so price is the competitive variable. Treatment courses of five years or more in the adjuvant setting give highly predictable repeat demand.
