IFA stands for iron and folic acid. The two are combined in one tablet because the body needs both to make healthy red blood cells. Iron goes into haemoglobin, the part of the blood that carries oxygen, and folic acid helps cells grow and divide, which matters most for a baby's development early in pregnancy. Taken regularly, IFA tablets help prevent and treat iron-deficiency anaemia.

Anaemia is still very common in India. In the National Family Health Survey 2019–21 (NFHS-5), the most recent national survey to measure anaemia, about 57% of women aged 15 to 49 and 67% of children aged 6 to 59 months were anaemic. That is why IFA tablets are handed out on such a large scale.

The red, blue and pink tablets

Since 2018, India's Anaemia Mukt Bharat ("anaemia-free India") programme has handed out IFA supplements free through schools, anganwadis and health centres. To make it easy to give the right dose to the right person, each group has its own colour:

The programme also includes regular deworming with albendazole, because worm infections are a common cause of anaemia in children.

What is inside an IFA tablet

Programme tablets usually use ferrous sulphate as the source of iron, together with folic acid, and are sugar- or film-coated, which masks the metallic taste and helps protect the tablet. Branded IFA products in pharmacies often use other iron salts, such as ferrous fumarate or ferrous ascorbate. When you compare products, look at the elemental iron, the amount of actual iron, rather than the weight of the salt.

  • Match the specification exactly. Government tenders in India follow the Anaemia Mukt Bharat specifications for strength, colour and packing, and other countries have their own. Small differences can disqualify a bid.
  • Check the label shows elemental iron, not just the salt.
  • Ask for stability data in hot, humid conditions. Folic acid is sensitive to heat and light, so ask for dissolution and folic acid results across the full shelf life too.
  • Get the documents up front: the manufacturer's WHO-GMP certificate, a Certificate of Analysis for each batch, and a CoPP if your market needs one.
