Emapalumab-lzsg is a fully human monoclonal antibody that binds and neutralizes interferon-gamma (IFN-γ), helping to suppress excessive immune activation. It is used in patients with hemophagocytic lymphohistiocytosis (HLH), a rare and life-threatening hyperinflammatory syndrome. By inhibiting IFN-γ activity, it helps restore immune balance in patients unresponsive or intolerant to conventional treatments such as corticosteroids, etoposide, or cyclosporine.

Emapalumab is a monoclonal antibody that binds interferon gamma and neutralises it, whether the cytokine is free or already bound to its receptor. In haemophagocytic lymphohistiocytosis, the immune system's off-switch fails: activated T cells and macrophages pour out interferon gamma, which drives further macrophage activation in a self-amplifying loop that causes fever, cytopenias, liver failure and, untreated, death within weeks.

It is used for primary HLH in adults and children with refractory, recurrent or progressive disease, or intolerance to conventional therapy — which is etoposide-based chemotherapy, effective but toxic, followed by bone marrow transplant. Emapalumab interrupts the cytokine loop directly rather than killing the activated cells, which is the conceptual difference. It is given by intravenous infusion twice weekly with dexamethasone, as a bridge to transplant. Because interferon gamma is central to defence against intracellular organisms, infection risk rises, and prophylaxis against tuberculosis, herpes zoster and Pneumocystis is given.
