Domperidone is a dopamine D2-receptor antagonist that does not readily cross the blood-brain barrier. It acts on the chemoreceptor trigger zone, which lies outside that barrier, and on the upper gut, where it improves stomach contractions and speeds gastric emptying. Because little reaches the brain, movement disorders are much rarer than with metoclopramide. In the EU, after a safety review, domperidone is limited to relief of nausea and vomiting. Adults and adolescents of 35 kg or more take 10 mg up to three times daily, maximum 30 mg a day, normally for no longer than one week. These limits reflect a small risk of QT prolongation, ventricular arrhythmia and sudden cardiac death. It is contraindicated with potent CYP3A4 inhibitors or QT-prolonging drugs, in moderate or severe liver impairment and in significant heart disease. It is not approved in the US.
