Disulfiram is an alcohol-deterrent medicine. It irreversibly inhibits aldehyde dehydrogenase, the liver enzyme that clears acetaldehyde, the first breakdown product of alcohol. If the patient drinks, acetaldehyde builds up within minutes, causing flushing, throbbing headache, nausea, vomiting, palpitations and a fall in blood pressure; severe reactions can include collapse. The tablets on this page contain 250 mg. Enzyme activity returns only as new enzyme is made, so the effect can last up to two weeks after the last dose.

Disulfiram is used in alcohol dependence for people who have stopped drinking and want support to stay abstinent, always within a programme of counselling and follow-up. It is never given covertly, and treatment starts at least 24 hours after the last drink. Patients avoid hidden alcohol in mouthwashes, some liquid medicines and toiletries. Liver function is checked because hepatitis, though rare, can be severe. It interacts with metronidazole, warfarin, phenytoin and isoniazid, and is avoided in heart disease and psychosis.

Sourcing notes. Disulfiram is prescription-only but not a controlled drug in most markets. Ask for a CTD or ACTD dossier with dissolution and bioequivalence data, or a justified waiver where your regulator allows one. Tablets are packed in blisters or tightly closed bottles, protected from light and moisture, at room temperature; ask for zone IVb stability for hot humid markets. The CoA per batch should cover assay, related substances, dissolution and uniformity of dosage units. Request the GMP certificate and a CoPP, and state plain or dispersible tablets, pack size and leaflet languages.
