Diacerein is a slow-acting anti-osteoarthritic agent (SYSADOA) used to manage osteoarthritis and related joint disorders. It works by reducing inflammation and inhibiting cartilage degradation, helping to relieve pain and improve joint function over time.

Product Description:

Diacerein is an anthraquinone derivative that inhibits interleukin-1 beta, the cytokine most implicated in cartilage breakdown in osteoarthritis. It reduces the production of the enzymes that degrade cartilage matrix and appears to support proteoglycan synthesis. It is classed as a symptomatic slow-acting drug for osteoarthritis, meaning the effect builds over weeks rather than hours and persists for a period after stopping — a different profile from an anti-inflammatory taken for pain relief.

It is used for osteoarthritis of the knee, taken as a capsule with food. Onset is slow, so it is not useful for a flare and is generally combined with analgesia early in treatment. Diarrhoea is the characteristic adverse effect, affecting a substantial minority of patients, typically within the first weeks and often severe enough to stop treatment. Hepatotoxicity has been reported. Urine turns a harmless darker colour.

Sourcing notes. Off patent, with availability confined largely to Europe, Asia and Latin America — it has never been approved in the United States. European regulators restricted its use in 2014 following a safety review: it is contraindicated in patients over 65, not indicated for hip osteoarthritis, and must be stopped if diarrhoea develops.
