Clofarabine is a purine nucleoside antimetabolite used primarily for the treatment of acute lymphoblastic leukemia (ALL) in pediatric patients (1–21 years old) who have relapsed or are refractory after at least two prior regimens. It inhibits DNA synthesis and repair, leading to apoptosis in rapidly dividing malignant cells.

Product Description:

Clofarabine is a second-generation purine nucleoside analogue designed to combine the useful properties of fludarabine and cladribine while resisting the enzymatic breakdown that limits both. Its halogen substitutions make it stable to adenosine deaminase and to acid. Once phosphorylated inside the cell it inhibits ribonucleotide reductase and DNA polymerase, and it also acts directly on mitochondria to release apoptotic factors, which gives it activity against cells that are not actively dividing.

The approved indication is relapsed or refractory acute lymphoblastic leukaemia in patients aged one to twenty-one who have already failed at least two regimens. It is given by intravenous infusion over five days. Toxicity is significant: profound myelosuppression, febrile neutropenia, tumour lysis syndrome, hepatotoxicity with transaminase rises, and a capillary leak or systemic inflammatory response picture that can be life threatening. Renal function must be monitored closely.

Sourcing notes. Originator product, though patents have expired in several territories and limited generic entry has begun. A paediatric orphan indication means very low, lumpy volumes concentrated in a few transplant centres. Cytotoxic handling required; store vials at controlled room temperature and do not freeze.
