Product Description:

Voriconazole is a triazole antifungal that blocks lanosterol 14-alpha-demethylase, the enzyme fungi use to make ergosterol for their cell membranes. Without ergosterol the membrane fails. Its importance rests on its spectrum: it is active against Aspergillus species, and it displaced amphotericin B as first-line treatment for invasive aspergillosis, a disease that kills a large proportion of the immunocompromised patients who develop it.

It is used for invasive aspergillosis, for serious Candida infections including fluconazole-resistant species, and for Scedosporium and Fusarium infections. The patients are typically neutropenic after chemotherapy or immunosuppressed after transplant — which is why this product is often stocked alongside oncology supplies. Its pharmacology is demanding: metabolism is non-linear and varies with CYP2C19 genotype, so therapeutic drug monitoring is standard. Transient visual disturbances affect about a third of patients. Hepatotoxicity, QT prolongation, photosensitivity with a long-term skin cancer risk, and numerous drug interactions require management.
