Product Description:

Teniposide is a semisynthetic derivative of podophyllotoxin and a close relative of etoposide, differing by a single substituent on the sugar ring. That change makes it considerably more lipophilic and more potent in the laboratory, with tighter protein binding and slower clearance. Like etoposide it poisons topoisomerase II, stabilising the cut DNA strand so that the break becomes permanent.

Its established role is in childhood acute lymphoblastic leukaemia, particularly in refractory disease and in combination with cytarabine, and it has been used in neuroblastoma and central nervous system tumours. Myelosuppression is dose-limiting. The formulation contains polyoxyethylated castor oil and alcohol, so hypersensitivity reactions — hypotension, bronchospasm, flushing — occur during infusion and are managed by slow administration over at least half an hour with premedication in patients who have reacted before. Secondary leukaemia is a recognised late risk.

Sourcing notes. Off patent, but a narrow, largely paediatric indication has left very few manufacturers and the product has been discontinued in several markets. The concentrate must be diluted and used within a defined window, and it is incompatible with certain plastics, so non-PVC administration sets are specified. Cytotoxic handling; refrigerated storage.
