Tenapanor is a minimally absorbed small-molecule inhibitor of the sodium/hydrogen exchanger NHE3 in the intestines. It works by reducing sodium absorption and increasing intestinal fluid, helping relieve constipation in IBS-C. In patients with chronic kidney disease on dialysis, it helps reduce phosphate absorption from the gut, lowering serum phosphorus levels. Product Description:

Tenapanor is a minimally absorbed inhibitor of NHE3, the sodium-hydrogen exchanger on the luminal surface of the small intestine and colon. Blocking NHE3 reduces sodium absorption, so more sodium and water stay in the gut lumen, softening stool and speeding transit. It also tightens the junctions between intestinal cells, reducing paracellular phosphate absorption — which gives it a second, unrelated use.

It is approved for irritable bowel syndrome with constipation in adults and, as a separate product, for lowering serum phosphorus in adults on dialysis whose phosphate is not controlled by binders, where it is added to binder therapy. It is taken twice daily immediately before breakfast and dinner. Diarrhoea is the main adverse effect and can be severe. It is contraindicated in children under six because of dehydration deaths in juvenile animal studies, and avoided in children from six to under twelve.
