Motixafortide is a subcutaneous injectable drug used in combination with filgrastim to mobilize hematopoietic stem cells into the peripheral blood for collection and subsequent autologous transplantation in patients with multiple myeloma. It works by blocking the CXCR4 receptor, which releases stem cells from the bone marrow. This therapy is used in patients who require stem cell collection.

Product Description:

Motixafortide is a CXCR4 antagonist. Haematopoietic stem cells are held in the bone marrow by the interaction between CXCR4 on their surface and the chemokine CXCL12 in the marrow niche. Blocking CXCR4 releases that anchor, and the cells move into the bloodstream, where they can be collected by apheresis. Its long receptor occupancy gives a prolonged mobilisation window.

It is used with filgrastim to mobilise stem cells for collection and autologous transplantation in multiple myeloma. In its pivotal trial, a large majority of patients collected enough cells for two transplants in a single apheresis session, compared with a minority on filgrastim alone — fewer apheresis sessions means less time, cost and patient burden. It is given subcutaneously about ten to fourteen hours before apheresis. Injection site reactions are very common and can be marked, and severe hypersensitivity reactions have occurred, so premedication with an antihistamine and observation are standard.

Sourcing notes. Originator product, on patent, no generic. Use is confined to transplant centres, so demand is concentrated and predictable. It competes with plerixafor, which is off patent and much cheaper, so the case rests on fewer apheresis sessions. Supply is through authorised distribution under cold chain.
