Hyoscyamine is a tropane alkaloid that works by blocking muscarinic acetylcholine receptors, thereby reducing smooth muscle spasms and secretions. It’s used to relieve symptoms such as abdominal cramps, irritable bowel syndrome, bladder spasms, and certain other conditions like peptic ulcer disease and excess saliva. Because of its anticholinergic effects, side effects can include dry mouth, blurred vision, light sensitivity, constipation, and urinary retention.

Hyoscyamine is the levorotatory isomer of atropine and carries essentially all of its antimuscarinic activity. It blocks muscarinic acetylcholine receptors on smooth muscle and secretory glands, reducing gut motility and spasm, gastric and salivary secretion, and bladder contraction. It crosses the blood-brain barrier, so central effects are part of its profile.

It is used for abdominal cramping in irritable bowel syndrome and other functional gut disorders, as an adjunct in peptic ulcer disease, for bladder spasm, to reduce secretions, and in palliative care for noisy terminal respiratory secretions. Sublingual, immediate-release and extended-release forms allow fast or sustained effect. The antimuscarinic adverse effects limit dose: dry mouth, blurred vision, constipation, urinary retention, tachycardia, reduced sweating with heat intolerance, and confusion in older people. It is contraindicated in angle-closure glaucoma, obstructive uropathy, paralytic ileus and myasthenia gravis.

Sourcing notes. In the US, many hyoscyamine products have been marketed without full FDA approval under historical arrangements, and the regulatory position of individual products differs, which matters for importers and payers. Outside the US, hyoscine butylbromide is usually preferred and hyoscyamine registration is limited. The very low tablet strength makes content uniformity the key manufacturing control.
