Fosinopril is an angiotensin-converting enzyme (ACE) inhibitor used to treat high blood pressure (hypertension) and heart failure. It works by relaxing blood vessels, reducing the workload on the heart, and lowering blood pressure to prevent complications such as strokes, heart attacks, and kidney problems.

Fosinopril is an ACE inhibitor prodrug, converted to fosinoprilat, which blocks the conversion of angiotensin I to angiotensin II. Blood vessels dilate, aldosterone falls and blood pressure drops. What sets fosinopril apart from other ACE inhibitors is elimination: it is cleared by both the liver and the kidneys, and when one route is impaired the other compensates. It is therefore often used without dose adjustment in renal impairment, where most ACE inhibitors need reducing.

It is used for hypertension and for heart failure with reduced ejection fraction, where ACE inhibition reduces mortality and hospitalisation. Dry cough is the characteristic class effect and the usual reason for switching to an angiotensin receptor blocker. Angioedema is rare but potentially life-threatening. Potassium and creatinine are checked after starting and after increases, since a modest creatinine rise is expected and acceptable. It is contraindicated in pregnancy, previous angioedema and bilateral renal artery stenosis.

Sourcing notes. Off patent, with generics available, though registration has narrowed as ramipril, perindopril and lisinopril dominate. The dual elimination route is the argument for using it in patients with kidney impairment, and that is what a buyer is usually sourcing it for.
