Flucloxacillin, by its action on the synthesis of the bacterial wall, exerts a bactericidal effect on streptococci, except those of group D (Enterococcus faecalis), and staphylococci. It is not active against methicillin-resistant staphylococci.

Product Description:

Flucloxacillin is a narrow-spectrum penicillinase-resistant penicillin, and its narrowness is precisely its value. It is stable to staphylococcal beta-lactamase, making it a first-line agent for methicillin-susceptible Staphylococcus aureus — skin and soft tissue infection, osteomyelitis, septic arthritis and endocarditis — while leaving the wider flora largely undisturbed. In an era of stewardship, an antibiotic that treats one organism well is preferable to one that treats many adequately.

It is supplied as 250 and 500 mg capsules and as oral solution, and as powder for injection.

One safety point belongs in labelling. Flucloxacillin is a recognised cause of cholestatic hepatitis, which may appear weeks after treatment has finished and is more frequent in older patients and with prolonged courses. The delayed onset means it is often not attributed to the drug.

Sourcing notes. Flucloxacillin is used principally in the United Kingdom, Ireland, Australia, New Zealand and parts of Europe; the equivalent agents elsewhere are dicloxacillin and, in the United States, nafcillin and oxacillin. Registration precedent therefore follows those geographic lines. WHO classifies it under Access.
