Felodipine is a dihydropyridine calcium channel blocker with high vascular selectivity: it relaxes arterial smooth muscle at doses that have little direct effect on the heart, so it lowers blood pressure without depressing contractility. It is formulated as an extended-release tablet, since the immediate-release drug is cleared quickly and would cause sharp vasodilation.

It is used for hypertension and for stable angina. Adverse effects are those of the class: ankle oedema, which is dose-related and caused by capillary pressure changes rather than fluid retention, plus flushing, headache and palpitations. Gum overgrowth can occur with long-term use. Felodipine is the textbook example of the grapefruit juice interaction — grapefruit inhibits intestinal CYP3A4 and can raise blood levels several-fold, causing hypotension — so patients are told to avoid it. Strong CYP3A4 inhibitors and inducers have the same effect in either direction.
